Project Risk Management

During the initiation phase, project managers identify risks and enter the details into the allocated tool. Upon investigation we found that 72% of projects had no further risks entered in to the system after this initial entry. Where risks had been entered, the quality of the risk statement and the mitigation actions were poor, meaning it was unclear exactly what the risk was and what mitigations were.
